Illinois Black Panther Party chairman Fred Hampton (21) was shot twice in the head while asleep in bed during a pre-dawn police raid on December 4. Fellow Panther Mark Clark was also killed. The raid was coordinated by State's Attorney Edward Hanrahan with FBI COINTELPRO intelligence from informant William O'Neal, who had provided a floor plan of Hampton's apartment.

FBI COINTELPRO agents created and mailed forged letters purportedly from Black Panther Party members threatening the US Organization, and vice versa, to provoke violence between the two California groups. The operation contributed to the murders of four Black Panthers on UCLA's campus and elsewhere in 1969. An internal FBI memo called the results 'tangible results' of the program.

Throughout 1969, Chicago police raided Black Panther Party offices multiple times. A June raid destroyed the office and beat members. The December 4 raid killing Fred Hampton was the culmination of a COINTELPRO-directed campaign. Illinois State's Attorney Edward Hanrahan praised the December raid as brave police work; subsequent investigation revealed police fired nearly all shots.
